Smoking and Hair Transplants: How It Affects Your Results

If there is one lifestyle factor that has a clear, well-documented negative effect on hair transplant outcomes, it is smoking. The mechanisms are multiple and the evidence is consistent — smokers have measurably worse graft survival rates and slower recovery than non-smokers.

How Smoking Affects Hair Transplant Results

Reduced Blood Flow to the Scalp

Nicotine is a powerful vasoconstrictor — it narrows blood vessels and reduces blood flow to peripheral tissues including the scalp. Transplanted grafts depend entirely on blood supply for survival in the days immediately after implantation. Restricted blood flow increases the risk of graft failure.

Carbon Monoxide

Carbon monoxide from cigarette smoke binds to haemoglobin in the blood, reducing oxygen carrying capacity. This further impairs oxygen delivery to healing tissues.

Impaired Wound Healing

Smoking reduces collagen production and impairs the cellular mechanisms of wound healing. The result is slower recovery, higher infection risk, and worse scarring at extraction sites.

Increased Shedding

Some studies suggest that smokers experience more pronounced post-operative shedding and slower regrowth, likely related to the compromised blood supply during the critical early growth phase.

How Long Before Surgery Should You Stop?

Most hair transplant surgeons strongly advise stopping smoking for a minimum of 2 weeks before surgery and 4 weeks after. Ideally, cessation for at least 4 weeks before and 6 weeks after provides significantly better conditions for graft survival.

Vaping and Alternatives

E-cigarettes and nicotine replacement products still deliver nicotine — which is the primary vascular culprit — and should also be avoided in the immediate pre and post-operative period. Nicotine patches and gum carry the same vasoconstriction risk, though without the additional carbon monoxide exposure.
