Hair Transplant How Much Does It Hurt?

✎ anzal • 📅 11 June 2026 • ⏱ 2 min read

Pain is one of the most frequently asked-about aspects of hair transplant surgery. The honest answer: there is some discomfort — mainly during the anaesthetic injections — but the vast majority of patients are surprised by how manageable the experience is.

During the Procedure

The Anaesthetic Injections

The most uncomfortable moment is the initial local anaesthetic injection. Most patients describe it as similar to a dental injection or a wasp sting — a sharp stinging sensation for 1–3 minutes. Once the anaesthetic takes effect, you should feel nothing more than pressure and movement.

During Extraction

With the donor area fully numbed, extraction is painless. You feel vibration and movement — nothing sharp. Some patients fall asleep during this phase.

During Implantation

With the recipient area numbed separately, implantation is similarly comfortable. Top-up anaesthetic is given if any sensitivity develops — do not hesitate to ask your surgeon.

After the Procedure

Evening of Surgery

As the local anaesthetic wears off over several hours, mild discomfort and tightness develop — similar to a sunburn sensation over the scalp. Paracetamol manages this well for most patients. A small number require codeine for 24–48 hours.

Days 2–5

Discomfort reduces significantly by day 2. Most patients describe mild tightness and aching rather than pain. The donor area can feel more tender than the recipient zone, particularly when lying back against a pillow.

Beyond Day 5

Pain is generally not a factor beyond the first week. Itching becomes the primary source of discomfort from week 1–3.

Anxiety About Pain

If anxiety about pain is a significant concern, discuss this at your consultation. Oral anxiolytic medication can be prescribed to help you relax on the day. The procedure is not performed under general anaesthetic and sedation is not standard, but some clinics offer oral sedation for anxious patients.
