Hair Transplant for Women: Am I a Good Candidate

Hair transplant surgery for women is an area of growing awareness and growing candidacy evaluation, but it remains a more complex clinical picture than for men. Understanding what makes a woman a good candidate for a hair transplant — and what disqualifies her — is important for setting appropriate expectations.

Female hair loss is most commonly androgenetic alopecia — also called female pattern hair loss or Ludwig pattern loss — which presents differently from male pattern baldness. Rather than recession from the temples and crown, female androgenetic alopecia typically produces diffuse thinning across the top of the scalp while the hairline remains relatively intact. This difference matters for surgical planning.

The key challenge in female hair transplant candidacy is donor stability. In women with diffuse androgenetic alopecia, the hair loss often affects the donor area as well as the top. If the donor zone — the back and sides of the scalp — shows miniaturisation on assessment, grafts taken from there will be subject to the same pattern loss after transplantation, and the result will not be permanent. This is the most common reason women are not suitable for hair transplant surgery when they approach a reputable clinic.

Women who are good candidates for hair transplant surgery typically have: a stable donor area (confirmed by dermoscopy showing no miniaturisation in the donor zone), patterned loss rather than diffuse loss throughout the scalp, and stabilised or slowly progressive androgenetic alopecia. Women with hairline recession due to traction alopecia or scarring alopecias in stable remission may also be appropriate candidates.

Alopecia areata, telogen effluvium, and other reactive or inflammatory hair loss conditions are not candidates for transplant surgery until the underlying condition is treated and stable.
