Can Minoxidil Regrow Lost Hair

Minoxidil is one of the most commonly used treatments for hair loss, and patients frequently ask whether it can actually regrow hair that has already been lost — not just prevent further loss. The answer requires careful distinction between different states of hair loss.

Minoxidil can stimulate regrowth in follicles that have miniaturised but not yet been destroyed. In androgenetic alopecia, follicles undergo a progressive miniaturisation process — producing finer and shorter hairs over successive cycles — before eventually becoming dormant. Minoxidil, by extending the anagen phase and increasing blood flow to the follicle, can partially reverse miniaturisation in follicles that are still viable but underperforming. The result is visibly thicker, more pigmented, longer hairs from follicles that were previously producing microscopic vellus hairs.

Minoxidil cannot regrow hair from follicles that have been completely destroyed and replaced by scar tissue. In very long-established baldness — a perfectly smooth, shiny scalp with no visible follicular openings — the follicles are gone, and minoxidil has nothing to work with. The longer the area has been bald, the lower the probability of meaningful minoxidil response.

The clinical trial evidence for minoxidil regrowth (rather than just maintenance) shows approximately 40% of users experience visible regrowth at the vertex after twelve months of twice-daily topical use. Regrowth at the frontal hairline is less consistent than at the crown.

The most important message about minoxidil regrowth is timing: the earlier in the miniaturisation process you start, the more responsive your follicles will be. Starting when miniaturisation is early produces better responses than starting when follicles are approaching the end of their functional life.
