Can Stress Cause Permanent Hair Loss

The question of whether stress can cause permanent hair loss is one that deserves a specific answer rather than the vague yes-and-no response it often receives. The distinction matters for how patients respond to stress-related hair loss and whether they pursue treatment appropriate to the cause.

Psychological stress causes telogen effluvium — a temporary, reversible hair shedding that resolves once the stressor is removed or adapted to. This is the well-established, well-understood mechanism by which stress causes hair loss. It is not permanent. The follicles are not damaged; they are temporarily synchronised into the resting phase and regrow when the cycle normalises.

Stress does not cause androgenetic alopecia in people who are not genetically predisposed. No amount of psychological stress accelerates the DHT-driven miniaturisation process in follicles that do not express the androgen receptor sensitivity that underlies male and female pattern hair loss.

Where stress and permanent hair loss intersect is in people who already have androgenetic alopecia. A stress-induced telogen effluvium in someone with underlying androgenetic hair loss will shed both the normally cycling hair and some of the already-miniaturising hair. After the telogen effluvium resolves, the temporarily shed normal hair regrows — but the miniaturised androgenetic hair may not regrow to the same extent as before, because the miniaturisation process was already progressing and the effluvium may have accelerated the visible expression of loss.

The conclusion: stress causes temporary shedding that reverses. It does not independently cause permanent hair loss. But in those with genetic predisposition, a stress episode can unmask or accelerate the expression of androgenetic loss that was already progressing.
