FUE Hair Transplant Aftercare Guide

A complete week-by-week guide to recovery after FUE surgery at Harley Street Hairline — from day one through to your 12-month result.

What to expect at each stage

Day 1–2
Immediate recovery

The first 48 hours

The scalp will be tender and the recipient area may feel tight. Some pinpoint bleeding and swelling is normal. Grafts are fragile at this stage and must not be disturbed.

  • Do: Sleep with your head elevated at 45° to reduce swelling
  • Do: Take prescribed pain relief and antibiotics as directed
  • Do: Keep the recipient area dry — no washing yet
  • Avoid: Touching or scratching the transplanted area
  • Avoid: Alcohol and smoking for at least 7 days
  • Avoid: Bending forward or any activity that increases scalp pressure
Day 3–7
First wash & early healing

Introducing gentle washing

Hair washing begins from day 3 using the technique demonstrated at your procedure. The goal is to soften and remove scabs without dislodging grafts.

  • Do: Wash daily from day 3 using the diluted shampoo method provided
  • Do: Use lukewarm water — never hot
  • Do: Pat dry gently with a soft cloth — no rubbing
  • Avoid: Direct shower pressure on the recipient area until day 10
  • Avoid: Picking at scabs — they will fall away naturally
  • Avoid: Wearing hats or tight-fitting headwear over grafts
Week 2–4
Scabs clearing, shedding begins

Normal shedding — do not be alarmed

Scabbing resolves completely by week 2 in most patients. From week 2–4, transplanted hairs begin to shed. This is not graft loss — the follicles remain intact beneath the scalp and will regrow.

  • Do: Resume normal hair washing and light exercise from week 2
  • Do: Continue minoxidil or finasteride if prescribed
  • Avoid: Swimming pools, saunas, and steam rooms until week 4
  • Avoid: Heavy gym work or contact sports until week 4
Month 1–3
Dormant phase

The quiet period

This is the stage most patients find challenging. The transplanted area looks sparse as shed hairs have not yet regrown. This is entirely normal — the follicles are establishing blood supply beneath the surface.

  • Do: Resume all normal activities, including exercise and swimming
  • Do: Attend your 1-month and 3-month remote photo reviews
  • Avoid: Concern — the dormant phase is expected and temporary
Month 3–6
Growth begins

Visible progress

New hair growth emerges from month 3–4. Initially fine and light, the hairs thicken and darken through months 4–6. By month 6, significant improvement is visible and most patients feel comfortable without concealment.

  • Do: Attend your 6-month photo review
  • Do: Resume styling, products, and heat tools from month 4
  • Do: Continue any prescribed medical treatment
Month 6–12
Full result

The final result

Hair continues to thicken and mature through months 6–12. The full result is assessed at 12 months, at which point the density, texture, and hairline position are fully established. Transplanted follicles are permanently DHT-resistant and will continue to grow for life.

  • Do: Attend your 12-month photo review — this is your baseline record
  • Do: Discuss long-term medical treatment with DrQ to protect surrounding native hairs

What is normal vs what to report

Most recovery milestones are predictable. These are the signals to watch for.

Normal — no action needed

  • Redness and pinkness in recipient and donor areas
  • Scabbing over transplanted grafts (days 3–14)
  • Mild swelling of the forehead (days 2–5)
  • Itching as the scalp heals (weeks 1–3)
  • Shedding of transplanted hairs (weeks 2–6)
  • Sparse appearance during months 1–3
  • Fine, light regrowth appearing from month 3

Contact the clinic if you notice

  • Pus, significant weeping, or wound discharge
  • Fever above 38°C in the days following surgery
  • Unusual pain that is not relieved by prescribed pain relief
  • Large areas of donor scarring or raised keloid tissue
  • No growth visible at all by month 5

Common aftercare questions

When can I wash my hair after a hair transplant?
Gentle washing begins on day 3 post-procedure using the diluted shampoo technique demonstrated by the clinic. This softens and removes scabs without disturbing grafts. Normal showering resumes from around day 10.
When can I go back to work?
Most patients return to desk-based work within 3–5 days. If your work involves heavy lifting, outdoor exposure, or direct sun, 2 weeks is advisable.
When can I exercise after a hair transplant?
Light walking is fine from day 3. Gym work, cardio, and heavy lifting should be avoided for 2 weeks. Swimming and contact sports are avoided for 4 weeks to protect healing grafts and prevent infection.
Is hair shedding after a transplant normal?
Yes — entirely normal and expected. Transplanted hairs shed from weeks 2–6 in a process called telogen effluvium. The follicles remain intact beneath the scalp. Regrowth begins from month 3–4.
When can I cut or style my hair?
Gentle trimming with scissors is possible from month 1. Clippers can be used from month 3. Heat styling tools, dyes, and chemical treatments should be avoided until month 4 to allow the scalp to fully recover.
When can I fly after a hair transplant?
Short-haul flights are generally fine from day 5–7. Long-haul flights are best deferred until day 10–14. The dry cabin air can dehydrate the scalp, so keep the recipient area moisturised during flight.
