Hair Transplant Crown vs Frontal: Which Should You Prioritise

When donor supply is limited — as it often is — the question of whether to prioritise the crown or the frontal area is one of the most important decisions in hair transplant planning. The answer is almost always the same, but the reasoning matters.

The frontal area — hairline and temples — should be prioritised in virtually all cases where the patient has both frontal recession and crown thinning. The reasons are visual, psychological, and practical.

Visually, the frontal hairline is the most seen and most socially significant aspect of hair coverage. It is the area that frames the face, that others observe first in social interactions, and that the patient sees most clearly in mirrors and photographs. A restored frontal hairline changes appearance more dramatically than crown coverage, because the crown is visible primarily from above — a perspective that is rarely the dominant one in social situations.

Psychologically, patients who restore the frontal area consistently report greater satisfaction with their result than those who address the crown first. The front-facing improvement is daily and constant; the crown improvement is intermittent and angle-dependent.

Practically, crown procedures require significantly more grafts per unit of visual coverage than frontal procedures, because the whorl pattern means grafts radiate outward — those at the centre have high visual impact, those at the periphery have diminishing returns. A frontal procedure using 2,000 grafts typically produces a more visually impactful result than a crown procedure using the same number.

The exception: patients whose frontal hairline is maintained and who have significant crown thinning as their primary concern. In this case, addressing the crown makes clinical and aesthetic sense.
