Dermaroller for Hair Loss: Does It Help

Microneedling — using a dermaroller or dermapen device to create micro-injuries in the scalp — has been investigated as a potential adjunct therapy for androgenetic hair loss. Understanding what the evidence shows helps distinguish genuine benefit from marketing claims.

The proposed mechanism is that micro-injuries stimulate the release of growth factors (PDGF, VEGF, EGF) in the scalp, triggering a wound-healing response that activates hair follicle stem cells and extends the anagen phase. This mechanism is biologically plausible and mirrors the rationale for PRP therapy.

The clinical evidence for microneedling in hair loss is modestly positive. Several randomised controlled trials have shown that adding microneedling to minoxidil treatment produces significantly greater improvements in hair density than minoxidil alone. The combination appears to enhance minoxidil absorption into the scalp and may independently stimulate follicle activity.

Used as a standalone treatment without minoxidil, the evidence for microneedling is weaker. It is best considered an adjunct that enhances the efficacy of topical treatments rather than a primary therapy on its own.

For patients who want to explore microneedling as part of a home hair loss regimen, a 0.5mm roller used once weekly on a clean scalp (no products for at least four hours before and after) is the commonly recommended approach. Sterilisation of the device is critical — using an unclean roller on the scalp carries a real infection risk. Clinician-administered microneedling sessions using higher needle depths offer more consistent results than home devices.
