Finasteride 1mg — sold under the brand name Propecia — is the most clinically proven oral medication for male pattern hair loss. Understanding how it works and how it fits alongside a hair transplant is important for anyone planning surgical hair restoration.
How Finasteride Works
Finasteride inhibits the enzyme 5-alpha reductase, which converts testosterone to dihydrotestosterone (DHT). DHT is the primary driver of androgenetic alopecia — it binds to receptors in genetically susceptible follicles, causing them to miniaturise and eventually cease producing terminal hair. By reducing DHT levels by approximately 70%, finasteride slows or halts this process.
Evidence for Finasteride
Clinical trials show that approximately 83–90% of men taking finasteride for 2 years experience a halt to further hair loss. Around 65% experience some degree of regrowth. It works best in the crown and mid-scalp — less reliably in the frontal hairline, where DHT sensitivity is highest.
Finasteride and Hair Transplant Together
A hair transplant does not require finasteride to succeed — transplanted hairs are DHT-resistant regardless. However, finasteride is often recommended alongside surgery to:
- Preserve native hair surrounding the transplanted zone
- Prevent the transplanted area from becoming an island if native hair continues to thin
- Potentially thicken existing miniaturising hair, improving density without additional grafts
Side Effects
The most discussed side effect is sexual dysfunction — reported in approximately 2–4% of patients in clinical trials. Most side effects resolve on stopping the medication. A very small number of patients report persistent side effects after cessation (Post-Finasteride Syndrome) — though this remains under scientific investigation.
Should You Take Finasteride?
The decision is personal and should be made in consultation with a doctor. For many patients, the benefits clearly outweigh the relatively low risk of side effects. Starting at the earliest signs of loss and continuing long-term achieves the best results.
