Graft Survival Rate: What Determines Success

The survival rate of transplanted grafts is one of the most important factors determining the quality of a hair transplant result. Understanding what affects graft survival helps patients evaluate clinics more effectively and manage the post-operative period correctly.

Graft survival depends on how quickly and carefully grafts are extracted, handled, stored, and implanted. Every follicular unit removed from the donor area begins a clock — the longer it remains outside the body, the lower its survival probability. The target for all quality practices is to minimise time out of body and maximise the conditions that keep grafts viable during that time.

The storage solution matters. Grafts stored in chilled saline solution remain viable for several hours. Enhanced storage solutions such as HypoThermosol maintain viability longer and are used by clinics that perform larger sessions. Temperature during storage — close to 4 degrees Celsius — is critical.

Handling is arguably more important than storage. Each graft must be gripped at the fat base rather than the follicle itself. Even brief, gentle compression of the follicle structure damages cells. Experienced technicians working under magnification produce significantly fewer damaged grafts than lower-skill handling.

Implantation technique matters too. Grafts must be placed at the correct depth and angle. Too deep, too shallow, or at the wrong angle reduces both survival and naturalness of outcome. The implantation phase is arguably the most skill-dependent part of the procedure.

In a well-run clinic with experienced surgical teams, graft survival rates of 90 to 95% are achievable and should be expected. A significant portion of underwhelming results from high-volume, low-cost clinics reflect survival rates below 70%.
