What Is the Difference Between Hair Loss and Hair Shedding

Hair loss and hair shedding are terms that patients often use interchangeably, but they describe fundamentally different phenomena with different implications for treatment and prognosis. Understanding the distinction prevents unnecessary anxiety and helps patients seek the right type of help.

Hair shedding is a normal, constant process. The average scalp contains approximately 100,000 hairs, of which 85 to 90% are in the anagen (growth) phase at any time and 10 to 15% are in the telogen (resting) phase. The telogen hairs are shed naturally — typically 50 to 100 per day in healthy adults — as they complete their resting phase and are replaced by new anagen growth from the same follicle. This shedding is noticeable in the shower drain, on pillows, and in the comb, and is entirely normal.

Hair loss, in the clinical sense, describes a net reduction in hair density resulting from either: follicles being permanently destroyed (as in scarring alopecias or very advanced androgenetic alopecia); follicles undergoing miniaturisation that reduces the calibre and length of produced hair (androgenetic alopecia); or a dramatic increase in shedding rate that outpaces regrowth (severe telogen effluvium).

The way to tell the difference: normal shedding does not change the overall density or appearance of your hair over time. Hair loss is characterised by a progressive change in density, coverage, or hairline position that is visible on comparison with photographs from months or years earlier.

If you are noticing increased shedding but your overall density looks unchanged, the most likely explanation is increased shedding awareness rather than a genuine increase — anxiety about hair loss makes people count every shed hair. If your density is visibly reduced, clinical assessment is warranted.
