Hair Transplant and Autoimmune Conditions

Autoimmune conditions that affect hair — including alopecia areata, lupus erythematosus, lichen planopilaris, and frontal fibrosing alopecia — present specific challenges for hair transplant surgery that require careful individual assessment before any surgical intervention is considered.

The fundamental challenge is the same across autoimmune hair loss conditions: the immune attack that caused the original hair loss can affect transplanted grafts in the same way as native hair. Transplanting into an environment of active autoimmune activity risks losing the transplanted grafts to the same immune process — wasting donor supply and potentially causing additional scalp trauma.

Stable, long-standing remission is the minimum prerequisite for surgical consideration in autoimmune hair loss. The definition of remission varies by condition — for alopecia areata, typically two years with no new activity; for scarring alopecias like lichen planopilaris, typically five or more years of stability with no evidence of active scarring on examination and dermoscopy.

Scarring alopecias are particularly challenging: the fibrosis that replaces destroyed follicles also reduces the vascularity of the recipient tissue, producing lower graft survival rates. The assessment must include evaluation of recipient tissue quality as well as disease activity.

Before considering any hair transplant in the context of an autoimmune condition, a current specialist opinion from a dermatologist familiar with your specific condition is essential. The surgical team needs to understand the disease state, the treatment history, and the risk of relapse before planning any procedure.
