Before-and-after photographs are the currency of the hair transplant industry. Every clinic has them. Not all of them are what they seem. Learning to evaluate what you are looking at — and understanding what is genuinely achievable — is essential before you commit to surgery.
How Before-and-After Photos Are Sometimes Misleading
- Lighting differences: Harsh overhead lighting in the “before” photo makes thinning look worse than in a flattering “after” shot
- Hair length and styling: Wet hair or a shaved head in the “before” vs styled hair in the “after” creates a false impression
- Timing: Photos taken at 6 months rather than 12–18 months miss the full result — or miss failure to thicken further
- Cherry-picking: Only the best results are shown; patients with average or poor outcomes are never featured
What to Ask When Viewing Results
When a clinic shows you results: ask for the graft count and Norwood Stage. Ask when the “after” photo was taken. Ask to speak with a real patient. Request examples of cases similar to yours, not just the most dramatic transformations.
What a Genuinely Good Result Looks Like
At 12–18 months, a well-executed FUE transplant should show: a natural hairline with soft, graduated edges; density that looks convincing under normal lighting and at conversational distance; no visible scarring in the donor area; and a result that blends seamlessly with existing native hair.
Density Expectations
Transplanted areas will not achieve the density of unaffected scalp. Aim for coverage that looks natural — not a hairline that would stand up to scrutiny under a medical microscope. The visual test is: does it look natural in everyday life? Not: does it look identical to age-18 hair under a magnifying glass?
