Hair Transplant for Black Men: What to Know

Men with African or Afro-Caribbean heritage represent a growing proportion of hair transplant patients in the UK, and their specific hair characteristics and concerns deserve dedicated attention rather than a generic approach to surgery.

The most significant technical consideration is follicle curl. Afro-textured hair follicles have a curved or tightly curled structure beneath the scalp surface, which means the straight FUE extraction punches used for straight hair have a higher transection (cutting through the follicle) rate when used with standard technique. Surgeons experienced in Afro hair FUE use modified punch designs and different extraction approaches to follow the follicle’s curved path and reduce transection rates to acceptable levels.

Despite the technical challenges, the visual outcomes of successful Afro hair transplants are often outstanding. The natural coil and volume of Afro-textured hair provides excellent visual coverage per graft — fewer grafts are typically needed to achieve the same perceived density as would be required for fine straight hair.

Keloid and hypertrophic scarring — more prevalent in individuals with darker skin types — should be specifically discussed before any surgical procedure. A history of keloid formation is not an absolute contraindication to hair transplant, but it influences the choice of technique and the specific approach to the donor area.

Hairline design for Afro hair has distinct aesthetic considerations. The natural hairline shape, temple point position, and ideal hairline height differ from European hair norms and should be designed with reference to what is natural for the patient’s specific background and facial structure.
