Hair Transplant for Crown Baldness: What to Know

Crown hair loss is one of the most common presentations for hair transplant surgery, and it is also one of the most nuanced to plan correctly. The crown presents specific challenges that differ from frontal hairline restoration, and patients considering crown procedures benefit from understanding these differences.

The crown’s whorl pattern means hair radiates outward in multiple directions from a central point. This creates a challenge for recipient site creation — each graft must be placed at the angle and direction corresponding to its specific position in the whorl, which changes continuously across the crown. A technically demanding implantation pattern that requires experience specific to crown work.

The coverage efficiency of grafts in the crown is lower than in the frontal area. Because hairs radiate outward from the centre, the central grafts have high visual impact but the peripheral grafts provide diminishing coverage per graft. Large crown areas require more grafts per unit of visual improvement than frontal procedures of equivalent size.

The progressive nature of crown loss is the most important planning consideration. Many patients who present with crown thinning have not yet reached their final Norwood Stage — the crown can continue to expand for years. Transplanting the centre of the crown with a high graft investment in a 35-year-old who progresses to Norwood 6 can leave an isolated central tuft surrounded by bald scalp — the opposite of what was intended.

For most patients under 45 with crown loss, medical therapy to slow the progression combined with a conservative approach to surgical intervention — focusing on the frontal area first — is the appropriate sequence. Crown coverage as a first procedure in a young patient with active progression is rarely the right plan.
