Hairline recession is the most common presentation in patients seeking hair transplantation — and the most satisfying to treat. A restored frontal hairline has an immediate and dramatic impact on appearance, framing the face and restoring a more youthful proportion.
What Causes Hairline Recession
Male pattern hairline recession is caused by DHT sensitivity in frontal follicles. The temples typically recede first, followed by the central hairline, creating the characteristic M-shape associated with Norwood 2–4 loss. The progression rate varies significantly between individuals.
How Hairline Restoration Works
DHT-resistant follicles from the permanent donor zone are implanted into the receded areas. The surgeon designs a new hairline that is natural, age-appropriate, and accounts for future loss trajectory. Single-hair grafts are placed along the very front for a soft, graduated leading edge.
Graft Requirements
For isolated hairline recession at Norwood 2–3, 800–2,000 grafts are typical. The exact count depends on the depth and width of recession and the desired hairline position. This is generally a single-session procedure.
The Result
A well-executed frontal hairline transplant looks completely natural. No straight lines, no plug-like appearance, no visible density variation at the edges. The transplanted hairs grow, respond to styling, and are entirely indistinguishable from native hair.
How Long Do Results Last?
Transplanted frontal hairline grafts are permanent. They will continue growing for life. However, native hair behind the transplanted zone may continue to thin if not protected with finasteride and/or minoxidil — making ongoing medical management important for younger patients.
