Hair Transplant Recovery Week 1: What to Expect Day by Day

✎ anzal • 📅 8 February 2026 • ⏱ 2 min read

The first seven days after your hair transplant are critical. Grafts are not yet fully anchored, and the scalp is healing. Following your surgeon’s aftercare instructions precisely during this window has a direct impact on your final results.

Day 1 — The Day of Surgery

You will leave the clinic with a light bandage over the donor area. The recipient area is left uncovered. Mild discomfort and a tight sensation are normal as the local anaesthetic wears off. Most patients manage well with paracetamol — avoid ibuprofen for the first 24 hours. Sleep with your head elevated (two pillows or a travel neck pillow) to reduce swelling.

Day 2–3 — First Wash

Most surgeons schedule a first gentle wash at 24–48 hours using a saline spray or diluted baby shampoo applied with a soft-spray bottle. Do not rub — gently pour and dab dry. Small crusts begin forming around each graft. This is normal and should not be disturbed.

Day 3–4 — Swelling Peak

Forehead swelling peaks at days 3–4 and can track down toward the eyes. This is caused by the infiltration fluid from anaesthetic and is normal. Sleeping elevated and limiting physical activity helps it resolve faster. It typically clears by day 5–7.

Day 4–6 — Crusting

Small crusts (scabs) are visible around each transplanted graft. These are protective and should not be picked or scratched. Continue gentle daily washing as instructed. The donor area is healing — small dots where grafts were extracted are crusting over.

Day 7 — End of Week 1

By day 7, most swelling has resolved. The donor area looks largely normal. Crusts remain on the recipient area but can be softened with gentle washing. Most patients feel comfortable returning to light desk work by day 5–7.

What to Avoid in Week 1

  • Touching, scratching, or picking at the transplanted area
  • Direct shower pressure on the scalp — use a gentle pour or spray only
  • Exercise or anything that raises blood pressure significantly
  • Alcohol and smoking
  • Direct sunlight on the scalp
