Hair Transplant Scars: FUE vs FUT — What Actually Shows

Scarring is one of the most common concerns patients raise before a hair transplant — and one where clinics do not always give the full, honest picture. Here is what FUE and FUT actually leave behind.

FUT scarring: what to expect

FUT (Follicular Unit Transplantation, also called the strip method) involves removing a linear strip of scalp from the donor area and dissecting it into individual grafts under a microscope.

This leaves a single linear scar across the back of the scalp. The scar:

  • Runs horizontally across the back of the head, typically 15–30cm long
  • Is 1–3mm wide in most cases when closed well
  • Is hidden by hair at normal lengths
  • Becomes visible when hair is cut very short (grade 1–2)
  • Is permanent and does not fade significantly over time

For patients who plan to keep their hair at a reasonable length, FUT scarring is generally not a practical concern. For patients who want to shave their head or wear very short styles, it is a significant limitation.

FUE scarring: what to expect

FUE (Follicular Unit Extraction) removes individual follicular units one by one using a small circular punch, typically 0.7–1.0mm in diameter.

This leaves multiple tiny circular scars scattered across the donor area rather than one linear scar. In practice:

  • Each scar is typically 0.5–1.0mm — barely visible individually
  • At normal density extraction (less than 50% of donor area), scars are effectively invisible at all hair lengths
  • Over-harvesting can leave visible diffuse scarring across the donor area
  • The scars are permanent but far less conspicuous than an FUT linear scar

Which is better for someone who wants to shave their head?

FUE — without question. The scattered micro-scars of FUE are virtually undetectable at grade 1–2. An FUT linear scar at the same length would be visible.

This is the primary practical reason FUE has become the dominant technique in the UK market.

Does FUE leave any visible scarring?

In the hands of an experienced surgeon using appropriate punch sizes and extraction density, FUE leaves no visible scarring at normal hair lengths. Over-harvesting — taking too many grafts from too small an area — is the main cause of visible FUE donor scarring.

This is why graft yield should always be balanced against donor preservation. Taking 4,000 grafts in a single session may produce a more dramatic immediate result but can leave the donor area visibly thinned — particularly in patients with fine or light-coloured hair.

What about recipient area scarring?

The small incisions made in the recipient area (where grafts are placed) heal without visible scarring in the vast majority of patients. The skin of the scalp heals well, and at 12 months there is typically no visible trace of the implantation sites.

The honest summary

  • FUT: One linear scar, hidden at most hair lengths, visible when shaved
  • FUE: Multiple micro-scars, effectively invisible at all hair lengths when performed correctly
  • Both: Permanent, but practically negligible for most patients with experienced surgeons

At Harley Street Hairline, we perform FUE exclusively. The technique offers the best cosmetic outcome with the least donor area compromise for the majority of patients.
