Best Sleeping Position After Hair Transplant

How you sleep in the first week after a hair transplant is more important than most patients realise. The right position reduces swelling, protects new grafts, and keeps the donor area comfortable. Here is what your surgeon should tell you.

Sleep Elevated for the First Week

Sleeping with your head elevated at approximately 45 degrees significantly reduces post-operative swelling. This is the single most important sleeping instruction for the first 5–7 days. Options include:

  • Two standard pillows stacked under your head and upper back
  • A travel neck pillow (U-shaped) to support without pressing on the recipient zone
  • A recliner chair set to approximately 45 degrees
  • A wedge pillow under the mattress

Protect the Recipient Zone

In the first 10–14 days, avoid pressing the transplanted area against a pillow. Sleep on your back rather than on your side or stomach. If you naturally roll in your sleep, place a pillow or rolled towel on each side to discourage movement. A travel neck pillow helps keep your head centred.

The Donor Area

The donor area (back of the scalp) can rest against a pillow from night one — the extraction sites are small and do not require the same protection as the recipient zone. A clean pillowcase each night helps minimise infection risk.

After Week 2

From around day 10–14, grafts are sufficiently anchored that normal sleeping positions are safe. You can return to your natural sleep position without concern. If some crusting remains, continue gentle care but position restrictions can be relaxed.

Practical Tips

  • Use a dark pillowcase — spotting from the healing scalp can stain white linen
  • Keep the bedroom cool — sweating increases swelling and discomfort
  • Avoid alcohol before bed — it increases swelling and blood pressure
