Norwood 5 and 6 Hair Transplant: What Is Achievable

Advanced hair loss — Norwood 5 and 6 — presents the greatest planning challenges in hair transplant surgery. The area requiring coverage is large, the donor supply is finite, and the goal shifts from restoration to optimised coverage within realistic constraints. Understanding what is achievable at this stage leads to better decisions and more satisfied patients.

At Norwood 5 and 6, the scalp has lost coverage across most of the top — the hairline, mid-scalp, and much of the crown. Restoring this entirely to the density of Norwood 1 is not possible in most patients with a single procedure or even multiple procedures. The approach shifts to strategic coverage — prioritising the areas that matter most visually (typically the hairline and frontal third) and using remaining supply to add coverage where possible.

Most patients at Norwood 5 or 6 will require 3,500 to 5,000 grafts or more for meaningful coverage across the top. Not every patient has this supply available — a thorough donor assessment is the starting point for any realistic plan. Body hair transplant (BHT) can supplement scalp donor supply in some patients.

SMP (scalp micropigmentation) is worth considering as an adjunct or alternative at this stage. For patients whose donor supply is insufficient for comprehensive surgical coverage, SMP can add apparent density to treated areas, conceal areas that surgery cannot reach, and create an overall appearance that surgery alone might not achieve.

Outcomes at Norwood 5 and 6, when approached with honest expectations and careful planning, can be genuinely satisfying. The comparison with the starting point — not with a teenage head of hair — is the appropriate benchmark.
