How to Tell If Your Hair Loss Is Permanent

One of the most anxiety-inducing questions for anyone experiencing hair shedding is whether what they are losing is coming back. The good news is that distinguishing temporary from permanent hair loss is usually possible with the right information — though clinical assessment is always more definitive than self-diagnosis.

Temporary hair loss (telogen effluvium) typically has a clear trigger: an illness, significant stress, childbirth, crash dieting, major surgery, or a medication. It presents as diffuse shedding — affecting all areas of the scalp roughly equally — rather than following a pattern. The loss typically accelerates two to four months after the trigger event, peaks over several weeks, and then gradually resolves as new growth replaces the shed hairs.

The scalp in telogen effluvium looks normal on close inspection. There is no miniaturisation of the remaining hairs, no visible scalp skin, and the scalp structure feels and looks like normal skin. When you look at shed hairs, they typically have a white bulb at the root — this is the telogen club hair, indicating a hair that has completed its cycle.

Permanent androgenetic hair loss looks different. It follows a pattern — receding at the temples, thinning at the crown, or both. Close inspection of the affected area shows miniaturisation: some hairs are visibly finer and shorter than others, reflecting progressive reduction in follicle size. This miniaturisation is visible under a dermoscope and is the diagnostic hallmark of androgenetic alopecia.

The most reliable clinical tool for distinguishing the two is dermoscopy with a trained clinician. If you are unsure, a trichology assessment will give you a definitive answer.
