Telogen Effluvium: How Long Does It Last

Telogen effluvium — the diffuse hair shedding triggered by a physical or psychological stressor — is one of the most common and most anxiety-provoking hair loss conditions, and the question of how long it lasts is one of the most important for patients going through it.

Acute telogen effluvium: triggered by a discrete stressor (illness, surgery, childbirth, bereavement, severe psychological stress, rapid weight loss), acute telogen effluvium peaks approximately three to four months after the triggering event. Shedding then gradually reduces and hair regrows over the following three to six months. Total duration from onset to recovery is typically six to twelve months. Most patients see their hair return to its pre-effluvium state within twelve months of the triggering event.

Chronic telogen effluvium: defined as shedding that persists beyond six months without a discrete triggering event. This pattern is less well understood but may represent a constitutional tendency for the follicle cycling to become dysregulated, or may reflect an ongoing stressor (nutritional deficiency, chronic illness, persistent psychological stress) that is maintaining the effluvium. Investigation to exclude ongoing causes is appropriate before concluding that the effluvium is idiopathic.

Recovery is the expected outcome in acute telogen effluvium. The follicles are not destroyed — they are cycling in a temporarily dysregulated pattern. Regrowth begins as the cycle normalises. The most important management is correct diagnosis to exclude ongoing causes, and patience.

Anxiety about the shedding can itself maintain a stress response that prolongs the effluvium — a difficult cycle to break. Understanding that the condition is self-limiting and that regrowth is the expected outcome reduces this anxiety-maintenance loop.
