What is Traction Alopecia and Can It Be Reversed

Traction alopecia is hair loss caused by repeated mechanical tension on the hair follicles over an extended period. It is most commonly seen in women who regularly wear tight braids, weaves, extensions, or ponytails, and in men who wear very tight cornrows or dreadlocks. It is an entirely preventable condition, but if left unaddressed for long enough, it can cause permanent follicle damage that requires surgical treatment.

The pattern of loss typically follows the hairline at the temples and frontal areas — precisely where the tension from tight hairstyles is greatest. Early signs include hair miniaturisation, breakage, and tenderness at the hairline. In the early stages, the damage is reversible if the causative hairstyle is discontinued and the follicles are given time to recover.

Chronic traction — particularly from weaves or extensions maintained over years — can cause progressive follicle fibrosis, where the follicle is replaced by scar tissue and can no longer produce hair. At this point, hair regrowth without surgical intervention is not possible.

For patients with permanent traction alopecia, FUE hair transplant surgery to restore the affected hairline is an effective option. The key requirement is that the hairstyling practices causing the damage must be permanently discontinued — continuing to apply traction to transplanted follicles will damage them in the same way as the originals.

Results from transplant surgery for traction alopecia are generally very good, as the surrounding donor area is typically healthy and unaffected by the condition, and the recipient area has defined borders that make planning straightforward.
