Hair Transplant Traction Alopecia: Can Hair Be Restored?

✎ anzal • 📅 27 April 2026 • ⏱ 1 min read

Traction alopecia — hair loss caused by prolonged tension on the follicles from tight hairstyles — is an increasingly common condition affecting women and men who wear braids, dreadlocks, tight ponytails, or hair extensions over extended periods. Understanding when surgery is appropriate is important before seeking treatment.

How Traction Alopecia Develops

Repeated tension on hair follicles causes inflammation around the follicle, progressive miniaturisation, and eventually permanent follicle destruction in areas of sustained tension. The frontal hairline and temples are most commonly affected. Early traction alopecia — where follicles are inflamed but not yet scarred — is fully reversible by stopping the cause. Late-stage traction alopecia — where follicles have been permanently destroyed and replaced by fibrous tissue — requires surgical restoration.

Is Your Traction Alopecia Suitable for Transplant?

Surgery is only appropriate when:

  • The causative hairstyle has been discontinued for at least 12 months
  • Hair loss has been confirmed as stable — not still progressing
  • A dermatologist or hair specialist has confirmed scarring (permanent) rather than active inflammatory loss
  • Donor supply is adequate for the area to be restored

What Results Are Possible

When performed correctly on well-selected patients, hair transplant surgery for traction alopecia achieves excellent results. The frontal hairline and temporal areas most commonly affected are technically straightforward zones for experienced surgeons. Graft counts are often modest — 500–1,500 grafts — for typical presentations.

Post-Transplant Hairstyling

After transplantation, tight hairstyles should be permanently avoided in the restored areas. Continuing the same high-tension styling will inevitably cause the same damage to the transplanted follicles as it did to the originals.
