Hair Transplant Scarring: What You Need to Know

Scarring after a hair transplant is one of the most common concerns for patients, particularly those who wear their hair short. Understanding what scarring actually looks like in modern FUE procedures — and what the realistic expectations are — helps patients make informed decisions.

FUE leaves small circular dot scars at each extraction site in the donor area. The diameter of each dot corresponds to the punch size used — typically 0.7 to 0.9 millimetres. These dots are white or slightly pink when healed. At hair lengths of number two grade (6mm) or longer, they are essentially invisible. At number one grade (3mm), they may be faintly visible under very direct light. Shaving the head completely bald reveals the dot pattern clearly — though it remains significantly less visible than the linear scar of FUT surgery.

The recipient area — where grafts are implanted — heals without visible scarring in the vast majority of cases. The recipient sites are created with fine blades or needles, and the healing process in the scalp is efficient enough that the sites are indistinguishable from surrounding skin within a few weeks. If a patient develops keloid or hypertrophic scarring at recipient sites, this is typically related to a pre-existing tendency to scar abnormally rather than a technical issue with the procedure.

Patients with a personal or family history of keloid scarring should specifically discuss this with their surgeon before proceeding. The donor area is a higher risk site for keloid formation than the recipient area due to the tension on the healing tissue.
