Best Shampoo for Hair Loss: What Actually Helps

The hair loss shampoo market is enormous, and the gap between what products promise and what they can deliver is significant. Understanding what shampoos can and cannot do for hair loss helps patients spend wisely and maintain realistic expectations.

What shampoos genuinely do: cleanse the scalp, reduce sebum and product buildup, and — in the case of medicated formulations — address scalp conditions that can exacerbate hair loss. A clean, well-maintained scalp supports healthy hair cycling. Scalp inflammation from seborrhoeic dermatitis or excess sebum can worsen shedding — addressing this with appropriate shampoo has a genuine (if limited) benefit.

Ketoconazole shampoo (Nizoral, available OTC or prescription): the best-evidenced shampoo for hair loss. Ketoconazole has antifungal properties that reduce Malassezia (the yeast implicated in seborrhoeic dermatitis) and also has modest anti-androgenic effects at the follicle level. Several studies have shown improvement in hair density with regular ketoconazole shampoo use. Used two to three times weekly, it is a cheap, low-risk adjunct to primary medical treatment.

Caffeine shampoos: the evidence for topical caffeine stimulating follicle activity is limited to in-vitro studies and small trials. Results are inconsistent and effect sizes are modest at best. Caffeine shampoos are unlikely to harm and may provide some benefit — but the evidence does not justify paying premium prices for them.

Biotin, collagen, and keratin shampoos: applying these ingredients topically provides no meaningful benefit to the hair follicle (which is in the dermis, well below where shampoo penetrates). These ingredients in shampoo condition the hair shaft — they do not affect hair loss.
