Hair Transplant Forehead Reduction

Surgical forehead reduction — lowering the hairline to reduce the height of the forehead — can be achieved either through hair transplantation or through a surgical forehead reduction procedure. Understanding the differences between the two approaches helps patients choose the right method for their situation.

Hair transplant for forehead reduction: individual follicular units are placed below the existing hairline, extending it downward to reduce apparent forehead height. This is the appropriate approach for patients who want a modest reduction (1 to 2 centimetres) and who prefer to avoid the linear scar of surgical hairline lowering. The transplanted hairs grow naturally and the result is permanent. The limitation is that the degree of lowering achievable in a single session is constrained by the density and coverage that transplanted grafts can achieve — very aggressive lowering in a single procedure risks a transplanted area that looks thin or sparse.

Surgical hairline lowering (forehead reduction surgery): the forehead skin is surgically advanced by excising a strip of forehead skin above the hairline and advancing the scalp forward. This can achieve 2 to 4 centimetres of lowering in one procedure but leaves a linear scar at the new hairline. The scar heals well in most patients and is often concealed within the hairline — but it exists and requires healing time.

The choice between the two: for modest reductions and patients without natural hairlines that are difficult to advance surgically, hair transplant is typically preferred. For patients wanting significant reduction and who are comfortable with a linear scar, surgical lowering achieves more in a single session. A combination — surgical lowering supplemented by hair transplant in the transition zone — is used in some practices.
