Hair Transplant After Previous Procedure: Repair and Revision

Revision hair transplant surgery — performed to improve, correct, or build on a previous procedure — is a growing segment of the hair restoration market, driven partly by the large volume of procedures performed at lower-quality centres and partly by patients whose first procedure produced a good but incomplete result who want further improvement. Here is what repair and revision involves.

The most common reasons for revision: poorly designed hairline (too straight, too low, wrong direction, or inappropriate for age); insufficient density from low graft survival in the initial procedure; over-extraction in the donor zone from a previous procedure that leaves visible depleted areas; or simply the desire to build on a satisfactory first procedure with a second session to achieve greater coverage.

Assessment of a revision candidate differs from a first-procedure assessment because the prior procedure has changed the landscape. The donor zone must be assessed for remaining extractable grafts — previous extraction may have significantly reduced what is available. The recipient area must be assessed for the quality of previously placed grafts (their direction, spacing, and survival) and for scar tissue that may affect implantation.

Correcting a poorly designed hairline is one of the most technically demanding types of revision. Removing misplaced grafts by surgical excision and replacing with correctly placed ones, or supplementing a poorly distributed hairline with additional grafts to fill in unnatural patterning, requires specific expertise. Not every surgeon who performs primary procedures has the experience for revision work.

Before undergoing revision surgery, obtain an honest, independent assessment from a surgeon who did not perform the original procedure. The assessment should identify specifically what needs correction and what is achievable given the remaining donor supply.
