Post-Transplant Finasteride: Should You Take It After Surgery

One of the most important — and most frequently under-emphasised — conversations a surgeon should have with patients is about medical maintenance after a hair transplant. The procedure restores what has been lost, but it does not stop the underlying process of androgenetic hair loss in the surrounding native hair.

Finasteride is the most effective medical tool for slowing this ongoing loss. By reducing DHT levels by approximately 70%, it slows the miniaturisation of remaining native follicles in the recipient zone and elsewhere on the scalp. Patients who take finasteride after their transplant preserve more of their surrounding hair for longer, which in turn means the transplanted area continues to look naturally integrated rather than increasingly isolated.

Studies comparing patients who did and did not take finasteride after hair transplantation consistently show better long-term results in those who continued medical therapy. The transplanted grafts themselves do not require finasteride — they are DHT-resistant regardless. But the native hair around and between the grafts benefits significantly.

Many patients are reluctant to commit to a long-term daily medication, and this is understandable. The discussion should be honest: finasteride is not mandatory, but stopping it — or never starting it — accelerates the natural loss that will eventually contrast with the permanent transplanted hair. For younger patients with active hair loss, the case for post-transplant finasteride is particularly strong.
